When setting up a custom script in configuration management for an App, there is no ability to specify an address or port to use. I assume then that it will use the resource address and the default SSH or Windows Remote Management ports. Is there any way to have it use a different port (e.g., if the VMs are behind a NAT with port forwarding, the SSH port might not be 22)?

Custom scripts for SSH uses port 22.
And for windows its 5985 (default HTTP WinRM port).
Its not supported at the moment to change the default ports.

I haven't tried it myself, but there is an attribute "CLI TCP Port" on resources that – in the networking driver code – seems to be the port used when making a connection. The default value of 0 seems to mean to use the default port for the specified protocol (e.g. SSH, telnet). You could try setting that attribute.
If you have a console server, you can also set the console-related attributes:
We're not using these now, but we have successfully used them in the past (on CloudShell 7.1) to access our network devices over SSH via a Cisco terminal server.

I just saw that you're asking about Apps, not devices. I don't see any port attributes on App models, so I don't think my previous response will apply.
